To ensure safe and effective care, we require recent lab work (completed within the last 6 months) from your primary care provider prior to your visit. This includes: CBC, CMP, thyroid panel, lipid panel, and either a hemoglobin A1c or glucose level. Vitamin B12 and vitamin D levels are optional but may be helpful.
